We are committed to serving the community with a variety of health-focused services, including education, screenings, and outreach. These offerings aim to improve health outcomes and provide accessible resources for all.

Community Wellness

We are committed to serving the community with a variety of health-focused services, including education, screenings, and outreach. These offerings aim to improve health outcomes and provide accessible resources for all.

  • Health Education and Outreach: Covering topics like women’s health, nutrition, smoking cessation, and chronic disease management (e.g., cancer, heart disease, diabetes).
  • Confidential Health Screenings: Including Body Mass Index (BMI), blood pressure, and advance care planning.
  • Vaccinations: Offering COVID-19, flu, pneumonia, Hep A, and tetanus vaccines.
  • Pediatric Immunization Clinics: Providing support for children’s vaccinations.
  • Point-of-Care Testing (POCT): Includes Hemoglobin A1C, blood glucose, and cholesterol readings.
  • Community Events: Participation in health and resource fairs to bring services directly to the public.

Street Outreach

As part of the Community Wellness Program, the Street Outreach Initiative provides direct support to individuals experiencing homelessness, substance use disorder, and mental health challenges. By offering essential services and fostering connections to treatment, we strive to improve access to care and long-term wellness.

  • Direct Services: Wound care, ongoing psycho-social support, and referrals for treatment and resources.
  • Health Education: Covering harm reduction, substance use disorder, behavioral health, and mental health topics.
  • Professional Training: Onsite substance use disorder education and Stigma Training for medical providers and professionals.
  • Community Collaboration: Working with local partners to expand access to care and advocate for individuals facing complex challenges.

Stephanie Perry, RN CARN, Addiction Nurse Specialist and Team Leader of our Community Wellness Program is an experienced and dedicated healthcare professional with a career spanning over 25 years in human service and community street outreach. For the past 15 years, Stephanie has served as an Emergency Room Registered Nurse, providing critical care and support to patients in need.

Stephanie is driven by the goal of reducing the stigma surrounding substance use disorders, mental health challenges, and homelessness while optimizing health outcomes for underserved populations.

Tamara Silva

Community Health Worker I

Tamara Silva is a Certified Medical Assistant for over 5 years of experience, who functions in the Community Wellness Program as an Outreach Specialist. She is fluent in English, Portuguese, and Spanish. Her passions include building meaningful connections and improving wellbeing of our community. She has worked in both outpatient and inpatient settings and enjoys working with a diverse population of patients and is dedicated to providing compassionate care.

For over three decades, Giovanna Borges has been a passionate community outreach worker an advocate and educator. Her dedication to the needs of the community has grown into a broader commitment to supporting underserved individuals with essential needs and those experiencing substance abuse and homelessness. Now part of a Southcoast Community Wellness team, she strives to reduce stigma and address health disparities within these vulnerable populations under the guidance of our addiction nurse specialist.

Sara Lapointe

Community Health Worker II

Sara Lapointe is a Community Outreach Specialist and Peer Recovery Coach with over 4 years’ experience working with clients and connecting them to community resources to support their needs.  Sara is an advocate for mental health and overall wellness, currently deepening her expertise in a master’s Program for Mental Health Counseling while working to become a Licensed Mental Health Counselor (LMHC). With a profound passion for serving the unhoused population, Sara is dedicated to understanding their unique challenges and actively working towards solutions that promote dignity and well-being.

Host-Site and Outreach Opportunities

The Community Wellness Program will provide selected services, education, and outreach opportunities in partnership with a community-based organization, also known as a host-site. The standard hours of operation of the program are Monday – Friday from 8:00 am to 4:30 pm, with flexibility when requested to provide services during evening hours and on the weekend.
