What is Population Health?
Population health is all about improving the health outcomes of groups of people — not just one patient at a time. This includes:
- Looking at data across groups of patients to see patterns of health and illness
- Making sure people have access to healthcare when and where they need it
- Preventing illness before it starts
- Managing chronic conditions (like diabetes or high blood pressure)
- Addressing social factors that affect health, like food access, housing, and transportation
